FELINE HIGH-DOSE DEXAMETHASONE/ACTH STIMULATION TEST
This combined screening test appears to be helpful in the diagnosis of feline hyperadrenocorticism. It involves taking 3 blood samples over a four-hour period, and is reported to be less affected by non-adrenal illness.

PROTOCOL

1. Take a basal blood sample for coritsol estimation.

2. Inject 0.1mg/kg dexamethasone (Hi-Dose) i.v.

3. Collect a 2-hour post dexamethasone sample for cortisol estimation.

4. Immediately inject 0.125 mg of synthetic ACTH (Synacthen, Alliance Pharmaceuticals) i.v.

5. Collect a third blood sample for cortisol estimation 1 hour after the ACTH.

INTERPRETATION

Normal cats show at least 50% cortisol suppression after dexamethasone and a normal coritisol response after ACTH stimulation (up to 400 nmol/l).

Cats with HAC show little suppression after dexamethasone and in exaggerated response following ACTH.
